Description
The Communities In Charge initiative assists broad-based, community coalitions design and implement sustainable, health care delivery systems to improve access to low-income, uninsured individuals. Financial and technical assistance is provided to community models that demonstrate a fresh approach to the organization and financing of local care delivery, incorporate managed care, promote prevention and early intervention, and integrate a full continuum of health care services.
Grants were awarded in two phases
Phase One (Organization and Planning
Phase Two (Development and Implementation)
Twenty communities received Phase One: Organizational
& Planning Grants
beginning January 2000.
Phase Two: Development
& Implementation matching grants
were awarded in January 2001 to
fourteen communities upon successful completion of Phase One.
