WHAT IS COMMUNITIES IN CHARGE?

Communities in Charge is a competitive grants program funded by the

Robert Wood Johnson Foundation that provides funding and technical

assistance to help 12 communities design and implement new, or

     WHAT ARE THE PROGRAM GOALS?

  • Develop broad-based community consortia to address local coverage issues

  • Implement sustainable delivery systems that manage care, promote prevention and early intervention and integrate services for the uninsured
